What you will do: Deliver local and personalised social care services to vulnerable adults with learning disabilities and autism. Conduct thorough investigations and assessments, providing detailed, evidence-based reports. Assess and address individual needs in collaboration with Council policies and legislation, while coordinating with other relevant bodies. Develop comprehensive support plans, negotiate services, and authorise care packages within financial parameters. Monitor support plans to ensure services provided are of high standard and amend provisions as necessary. Advocate for and ensure the rights of those you support are upheld, maintaining dignity, respect, and self-determination at all times. Provide advice on and connect clients and carers with local services and support networks. Maintain accurate and up-to-date records using the department’s IT systems.
