What you will do

• Complete comprehensive assessments and care plans to aid the transition of individuals from temporary care facilities to their long-term providers.
• Work two days a week in the office, with the flexibility to conduct home visits as part of the support network.
• Participate actively in the duty rota, ensuring that colleagues and service users receive timely help and interventions.

Requirements

• Candidates must have a minimum of 2 years post-qualified experience in social work.
• Must not have held a permanent social worker post in the last 12 months.
• Be prepared to offer a 1-month notice upon accepting this role.
• Rate of £28.40 per hour will be paid via an umbrella company and is inclusive of HPIL; this rate is firmly set.
