What you will do:

• Undertake the assessment of patients and service users with complex needs, including those with an unconfirmed diagnosis.
• Develop, implement, and evaluate specialised programmes of care using the latest evidence and research to ensure the best outcomes.
• Work autonomously while recognising changes in patients or service users that require intervention, escalating appropriately.
• Guide and advise other staff in the assessment, planning, implementation, and evaluation of patient care.
• Promote involvement of patients and service users in their care plans, ensuring respect for their beliefs and dignity.
• Build strong therapeutic relationships with patients, service users, and their support networks, utilizing effective communication skills.
• Act as a role model for junior staff, promoting high standards of practice and professional values.
• Oversee the service as needed and coordinate multi-agency activities within your specialised area.
• Participate in service developments, propose changes to enhance practice, and lead MDT meetings.
• Ensure colleagues are up-to-date with relevant policies and nutritional care guidelines.
