Main purpose of the job:

• Manage a mixed caseload within our generic community social work teams, providing effective and person-centred assessments and interventions across Medway.
• Support adults with care and support needs alongside their families and carers to live fulfilling independent lives.
• Work with a preventative and enablement focus, delaying the need for long-term care where possible by leveraging individual strengths and support networks.
• Understand and operate within financial, legal, and organisational frameworks including Human Rights and Equalities legislation, the Care Act, Mental Capacity Act, and Mental Health Act.
• Prepare clear reports for strategy meetings and court proceedings.
• Liaise with stakeholders to promote our One Council Plan and our core values and behaviours.

What you will do:

• Work directly with adults to establish their wishes and feelings using a variety of techniques.
• Demonstrate and model good practice reflecting a commitment to relationship and strengths-based approaches.
• Manage a workload by engaging in reflective supervision, conducting strengths-based assessments, and developing person-centred plans.
• Participate in regular supervision sessions to review casework and identify learning areas.
• Stay updated with social work research findings and innovative practices.
• Seek advice from supervisors and other professionals to support decision-making processes.
