What you will do:
- Conduct thorough assessments of individuals' needs for care and support, planning and implementing solutions that promote independence and positive change.
- Develop, review, and reassess care plans to ensure the evolving needs of each individual are met effectively.
- Oversee and engage in adult safeguarding enquiries and collaborate with other professionals to develop safeguarding plans tailored to the wishes and circumstances of individuals.
- Utilize a strengths-based approach to foster self-determination and resilience in those you support.
