What you will do:

Conduct assessments, reviews, and reassessments under the Care Act 2014 to develop personalised care plans.
Respond to safeguarding concerns, ensuring risk assessments and protective measures are in place.
Perform Mental Capacity Assessments and contribute to decisions that uphold individuals' rights.
Manage a caseload, reviewing services and performing assessments to ensure care effectiveness.
Collaborate with health professionals, housing services, and other agencies to coordinate comprehensive care plans.
Maintain detailed, accurate case records in line with professional standards and agency policies.
Engage in ongoing supervision and training to foster professional growth.

Person Specification:

Degree in Social Work (BA / MA / DipSW).
Registration with Social Work England (SWE).
Experience within Adult Social Care in a local authority or statutory setting, particularly in Care Act assessments and safeguarding.
