Job Summary:
- Work within the Trust's CPA/Care Management framework, promoting high standards of patient care and communication.
- Act as a care co-ordinator for a defined caseload of clients with severe and enduring mental health problems.
What you will do:
- Manage a flexible caseload that meets the unique needs of clients and their carers.
- Provide high standard individualised client care, assessing mental, emotional, and physical well-being.
- Act as a care coordinator to assess client needs and ensure care plans are implemented and updated regularly.
- Participate in crisis intervention measures and manage the admission, discharge, and transfer process of clients.
- Supervise therapeutic treatments, ensuring compliance with Trust Policy and NMC Standards.
- Engage in weekly clinical meetings, collaborate with in-patient facilities, and monitor the efficacy of care plans.
