What you will do:

Undertake Care Act 2014 assessments, reviews, and reassessments, ensuring care and support plans are person-centred and promote wellbeing and independence
Identify, assess, and respond to safeguarding concerns, contributing to safeguarding plans and participating in multi-agency meetings
Conduct Mental Capacity Assessments, apply relevant legislation, and support individuals and families to understand their rights
Collaborate with health professionals, housing providers, and other partners to deliver coordinated support, attending multidisciplinary meetings and case discussions
Manage a caseload of adults with complex needs, ensuring commissioned services are of high quality and provide value for money
Maintain accurate and timely case records, participate in supervision and training, and contribute to service improvement initiatives

Person Specification:

Hold a degree in Social Work (BA / MA / DipSW) and be registered with Social Work England (SWE)
Experienced in Adult Social Care within a local authority, particularly in undertaking Care Act assessments and safeguarding
Desirable experience in working with adults with complex or long-term needs
