Job Purpose:
To contribute to the operational delivery of an effective Adult Social Care Service as part of an integrated team responsible for safeguarding, support planning and promoting the wellbeing of adults with care and support needs, continuously improving outcomes. Roll up your sleeves and dive into planning and co-designing individual support plans, ensuring regular monitoring and carrying out reviews. Manage your own caseload while keeping in line with the council's values and procedures.
What you will do:
1. Manage a complex and varied caseload with appropriate supervision and support.
2. Prepare written reports, statements, and representations for Court hearings.
3. Make complex decisions and manage risks on caseloads using your professional judgment.
4. Communicate confidently in complex or high-risk situations.
5. Attend and represent the department in multidisciplinary forums and act as Chair when required.
6. Conduct direct work with service users, carers, and families according to interagency safeguarding policies.
7. Make assessments and act as a care navigator for individuals with eligible social care needs.
8. Collaborate with various stakeholders to plan tailored responses to individual needs.
9. Ensure services meet assessed needs within financial constraints.
10. Maintain an effective system for the coordination, monitoring, and review of support plans.
11. Identify gaps in services or unmet needs and report to the line manager.
12. Facilitate student placements if qualified as a social worker practice educator.
13. Lead safeguarding enquiries and ensure outcomes are recorded and shared appropriately.
14. Work cooperatively within the council and across multi-agency boundaries.
