What You Will Do:
- Complete comprehensive Occupational Therapy assessments within service users' homes.
- Assess functional ability, risks, and independence in home environments.
- Recommend and facilitate minor and major adaptations via the Disabled Facilities Grant pathway.
- Provide tailored complex equipment solutions.
- Conduct manual handling assessments and recommend necessary equipment.
- Support adults to remain safe and independent in their homes.
- Manage a complex community caseload with diligence and care.
- Collaborate with service users, carers, and multidisciplinary professionals for seamless service delivery.
- Produce detailed assessment reports and recommendations to guide interventions.
- Monitor and review the outcomes of interventions and adaptations for effectiveness.
- Contribute to person-centred care planning and risk management strategies.
Requirements:
- HCPC registered Occupational Therapist.
- Experience assessing adults within home environments.
- Proficient in Adult Social Care Occupational Therapy and recommending necessary equipment and adaptations.
- Skilled in conducting manual handling assessments and managing complex caseloads.
- Thorough knowledge of the Disabled Facilities Grant process.
- Capability to complete detailed home environment assessments independently.
- Strong report-writing and communication skills.
Desirable Requirements:
- Experience with a Local Authority.
- Background in complex adaptations services, preferably within a West Locality or community OT team.
- Expertise in supporting DFG-funded adaptations.
