What you will do:
- Conduct comprehensive assessments of individuals within the community to understand their needs and aspirations.
- Develop person-centred care plans that effectively address mental health challenges and promote positive outcomes.
- Work collaboratively with a multidisciplinary team to provide holistic support to service users.
- Review and monitor service plans, ensuring they meet the evolving needs of individuals.
- Advocate on behalf of service users to ensure they receive the support and services required for their well-being.
- Maintain accurate and professional records, adhering to both organisational and regulatory standards.

This role is suited for experienced Adult Social Workers who hold a recognised Social Work qualification and are registered with Social Work England. You should have significant experience within a social care setting, particularly working with adults, and be adept at managing your workload whilst maintaining high-quality standards.
