What you will do:

• Develop and implement individualised active recovery and reablement plans.
• Maintain strong links across the county's three area clusters for seamless service delivery.
• Conduct home visits across the county as required.
• Ensure documentation and reporting are completed accurately and on time.
• Engage in meetings and training sessions to enhance service provision and personal development.

Requirements:

• You must be HCPC registered and possess a valid driver's licence to support your role in this rewarding venture.
