What you will do

Undertake Care Act 2014 assessments, reviews and support planning for adults requiring social care intervention following hospital admission.
Facilitate safe, timely and effective hospital discharges through collaborative working with health professionals, families, carers and community services.
Complete Mental Capacity Act assessments, Best Interest decisions and risk assessments where required.
Manage a caseload of adults with complex care and support needs, ensuring outcomes are person-centred and strengths-based.
Undertake safeguarding enquiries and contribute to risk management processes in accordance with relevant legislation and local procedures.

Person Specification

Degree in Social Work, CQSW, DipSW or an equivalent recognised Social Work qualification.
Evidence of continuing professional development relevant to Adult Social Care.
Current registration with Social Work England SWE.
Post-qualification experience within Adult Social Care.
Experience of working within a hospital discharge, integrated care, community social work or reablement service.
