What you will do

• Undertake comprehensive biopsychosocial and risk assessments for adults referred to the SPA Living Well Service.
• Triage referrals and determine appropriate pathways, ensuring timely access to community, primary care, voluntary sector or specialist mental health services.
• Provide clinical advice and support to GPs, Primary Care Networks (PCNs), social care teams, and partner agencies.
• Deliver brief evidence-based interventions, recovery-focused support, and crisis prevention planning where appropriate.

Qualifications and Experience

• Hold a Registered Mental Health Nurse (RMN) qualification.
• Current registration with the Nursing and Midwifery Council (NMC).
• Evidence of continuing professional development relevant to community mental health practice.
• Training in risk assessment, safeguarding, and mental health interventions.
• Experience within community mental health services, crisis pathways, SPA, CMHT, or Living Well services, managing individuals with severe and enduring mental illness and complex psychosocial needs.
