What you will do

• Conduct annual statutory reviews and provide support for reassessments and unplanned reviews of care in line with the Care Act 2014.
• Perform strengths-based assessments as well as support planning and risk assessments to foster independence and enhance wellbeing.
• Collaborate with health professionals, occupational therapists, housing providers, and voluntary organisations to coordinate and optimise support services.
• Identify any safeguarding concerns, carrying out enquiries when needed, and actively participate in safeguarding adult processes.

In addition to these responsibilities, you'll use your expertise and qualifications, such as a degree in Social Work or equivalent, to undertake complex assessments and deliver robust care solutions. With experience in UK Adult Social Care settings, you'll navigate the challenges of supporting adults with varying needs, from physical and learning disabilities to mental health requirements.
