What you will do:

• Undertake comprehensive occupational therapy assessments to identify functional needs, risks, and opportunities to maximise independence.
• Recommend and arrange appropriate equipment, adaptations, assistive technology, and moving and handling solutions.
• Complete assessments and reviews in accordance with the Care Act 2014 and relevant Adult Social Care legislation.
• Work collaboratively with social workers, NHS professionals, housing providers, carers, and community organisations to coordinate support plans.

Person Specification:

• Degree or Diploma in Occupational Therapy recognised by the Royal College of Occupational Therapists.
• Evidence of continuing professional development relevant to Adult Social Care practice.
• Current registration with the Health and Care Professions Council HCPC.
• Experience working as an Occupational Therapist within Adult Social Care, community services, rehabilitation, or health and social care settings.
• Experience of undertaking complex assessments and developing person-centred support plans.
