What you will do

Manage a varied caseload of service users, guiding them from initial engagement through to treatment, recovery planning, and their successful exit from services.
Conduct detailed assessments, risk evaluations, care planning, and ongoing reviews adhering to organisational policies and clinical governance.
Facilitate one-to-one and group psychosocial interventions focused on recovery, prevention of relapse, harm reduction, and overall wellbeing.
Advise and support on issues related to substance misuse, physical health, mental health, as well as assisting with housing, employment, and promoting social inclusion.
Collaborate effectively with prescribing clinicians, healthcare professionals, safeguarding teams, and community partners to develop and execute effective support plans.

Requirements

Current DBS on the update service
Demonstrated experience in substance misuse (drugs & alcohol)
Strong caseload management and keyworking skills
Comprehensive understanding of harm reduction and recovery models
