What you will do:

• Manage a caseload of adults with severe and enduring mental health conditions, delivering comprehensive assessment, intervention, treatment, and recovery support.
• Undertake mental health, risk, and safeguarding assessments to develop person-centred care plans that meet the unique needs of each client.
• Act as Care Coordinator under the Care Programme Approach (CPA), organising effective planning, review, and care evaluation.
• Provide evidence-based nursing interventions in community settings and monitor treatment outcomes to ensure optimal results.
• Administer, monitor, and evaluate medication while identifying and addressing any physical or mental health concerns.

Person Specification:

Qualifications:

• Registered Mental Health Nurse (RMN)
• Degree or Diploma in Mental Health Nursing
• Continuous professional development in community mental health practice

Registration & Compliance:

• Current NMC registration with no restrictions

Experience:

• Significant post-registration experience in community mental health services, CMHTs, or similar secondary care settings
• Expertise in working with adults presenting complex mental health needs, including psychosis, severe mood disorders, and dual diagnosis
