What you will do
- Assess individuals in their homes following their hospital discharge.
- Facilitate safe and timely discharges from hospital through the Discharge to Assess pathway.
- Complete functional assessments and conduct in-depth activity analyses.
- Recommend and prescribe adaptive equipment, assistive technologies, and minor modifications to improve patient independence.
- Perform moving and handling assessments and conduct regular reviews to ensure safety and care quality.
- Collaborate closely with multidisciplinary partners to develop and implement personalised support plans.
- Encourage independence and support reduced dependency on continuous care where feasible.
- Assist individuals, their families, and carers during periods of transition and recovery.
- Maintain comprehensive and timely case records to ensure high standards of care.
Requirements
- Valid HCPC registration.
- Driving licence.
