What you will do

• Assess individuals in their homes following their hospital discharge.
• Facilitate safe and timely discharges from hospital through the Discharge to Assess pathway.
• Complete functional assessments and conduct in-depth activity analyses.
• Recommend and prescribe adaptive equipment, assistive technologies, and minor modifications to improve patient independence.
• Perform moving and handling assessments and conduct regular reviews to ensure safety and care quality.
• Collaborate closely with multidisciplinary partners to develop and implement personalised support plans.
• Encourage independence and support reduced dependency on continuous care where feasible.
• Assist individuals, their families, and carers during periods of transition and recovery.
• Maintain comprehensive and timely case records to ensure high standards of care.

Requirements
