What you will do

  • Undertake strengths-based social work assessments and reviews for adults requiring support following hospital admission or discharge.
  • Coordinate and facilitate safe and effective hospital discharges through the Discharge to Assess model across acute, community, and out-of-area hospital settings.
  • Develop and implement person-centred care and support plans that maximise independence and well-being.
  • Work collaboratively within a multi-disciplinary team, including NHS clinicians, therapists, nursing staff, housing providers, and community organisations, to deliver comprehensive care.
  • Complete Mental Capacity Act assessments, Best Interest decision-making processes, and risk assessments where required.

Person Specification

Qualifications

  • Degree in Social Work, Social Care, CQSW, DipSW or equivalent recognised qualification.
  • Evidence of continuing professional development.

Registration & Compliance

  • Registered with Social Work England (SWE).

Key Skills

  • Strengths-based assessment and care planning.
  • Expertise in complex case management within hospital and community settings.
  • Ability to safeguard adults and manage risk effectively.

Experience

  • Proven experience within Adult Social Care, hospital discharge, community social work, or integrated healthcare settings.
  • Experience managing complex adult social care cases.
  • Strong history of working alongside NHS and community partners within multi-agency environments.
