What you will do

  • Conduct comprehensive Care Act assessments and reviews of adults' needs.
  • Develop person-centred and strengths-based care and support plans.
  • Review and reassess long-term care packages to ensure they deliver desired outcomes.
  • Work with individuals to maximise their independence using community and informal supports.
  • Undertake safeguarding duties and refer situations as needed to protect individuals from harm.
  • Lead on capacity assessments and best-interest decision-making where appropriate.
  • Collaborate with multi-agency partners to coordinate care and support services.
  • Maintain accurate case records, reports, and professional documentation to high standards.
  • Participate in ongoing training, supervision, and continuing professional development.
  • Stay abreast of relevant social care legislation and statutory responsibilities.
