What you will do:

- Carry out comprehensive assessments within the community for people with permanent or complex disabilities, ensuring the provision of services that prevent ongoing dependence.
- Perform assessments under various legislations such as the Care Act and Housing Grants, with a focus on reducing handoffs and promoting environmental adaptations that enhance independence in care provision.
- Collaborate as part of a multidisciplinary team to review and enhance the effectiveness of existing care, exploring paths for greater independence among service users.
- Supervise Occupational Therapy Assistants and students effectively, providing guidance and support where necessary.
- Engage in tasks that align with the OT and Sensory service's goals to support the Adult Social Care team's aspirations and vision.
- Partner with service users, carers, professionals, and other stakeholders to foster maximum independence.

Requirements:

- Registered with the Health and Care Professions Council (HCPC).
- Must hold a valid driving licence and be willing to drive as part of the role for community-based assessments.
