What you will do:
- Lead therapy assessments and rehabilitation of patients admitted into the service, primarily from hospitals. Your expertise will drive patient progress and recovery
- Conduct access visits to properties, determining the need for minor adaptations and provision of equipment to enhance patient independence
- Manage a varied caseload, delegating tasks effectively to therapy assistants while maintaining oversight of patient care
- Take on referrals as part of the wider discharge to assess pathway, contributing to seamless patient transitions
- Liaise with multidisciplinary team professionals to ensure streamlined and effective discharges
- A driving licence and access to a vehicle with appropriate business insurance is essential for effective community and home visits
Required:
- Registration with the HCPC is a must to practise and apply specialised knowledge in this role
