What you will do:

  • Manage a caseload of young people aged 17–25, undertaking Care Act assessments, reviews and support planning
  • Apply the 3 Conversations Model to deliver person-centred, strengths-based interventions focused on independence and resilience
  • Complete mental capacity assessments and contribute to Best Interest decisions in line with the Mental Capacity Act
  • Develop and commission care and support packages, including personal budgets and direct payments
  • Lead and participate in multi-agency meetings, including EHCP reviews, Section 117 aftercare planning, and Continuing Healthcare processes

Person Specification:

  • Degree or equivalent in Social Work (BA/BSc/MA, DipSW or CQSW)
  • Registered with Social Work England
  • Strong working knowledge of the Care Act 2014
  • Strong working knowledge of the Mental Capacity Act 2005
  • Strong working knowledge of the Equality Act 2010
  • Understanding of Safeguarding Adults procedures and statutory guidance
  • Familiarity with the Preparing for Adulthood framework
  • Familiarity with SEND processes
