This exciting opportunity offers a competitive salary of £28.40 hourly. You will work Monday to Friday from 8.45 to 17.15, totaling 37 hours a week. What you will do:

• Assess the needs of service users and develop outcome-focused, person-centred occupational therapy plans
• Co-produce care and support plans where necessary to ensure holistic care
• Provide expert advice on services, equipment, and support options available to promote independence and wellbeing
• Deliver and oversee reablement programs aimed at maintaining independence and enhancing quality of life
• Prescribe and arrange necessary equipment and adaptations to meet the specific needs of individuals
• Regularly review and reassess the support provided to ensure its effectiveness and make improvements as needed
• Collaborate with other professionals and agencies to provide comprehensive support
• Offer guidance and support to service users, their families, and carers
• Maintain accurate case records and utilise IT systems for efficient case management
• Participate in duty, team development, and performance tasks to contribute to the community’s well-being
• Maintain HCPC registration and adhere to set policies and standards

Requirements:

• Enhanced DBS (Adults and Children's Barred List) and DFE form required – all submissions without these will be rejected
• Candidates must be experienced in manual handling and optimal handed care, ensuring a high standard of occupational therapy
