What you will do

• Conduct comprehensive Continuing Healthcare assessments for adults with complex health needs.
• Manage comprehensive client case management for patients receiving Continuing Health Care funding, ensuring they achieve optimal function levels.
• Present assessments to the BSOL Continuing Care panels for ratification.
• Engage with health, social care, and independent sector colleagues to address and coordinate healthcare needs thoroughly.
• Maintain professional relationships with diverse partners, such as local authority partners, district nursing services, and palliative care teams.
• Provide accurate and timely reports and assessments, ensuring all care aligns with the highest standards of practice and policy.
• Take charge of planning and organisation, ensuring timely completion of healthcare assessment functions.
• Navigate through complex cases with legal and ethical accountability, bringing the best clinical judgment to assessment processes.
