Job Purpose

We are on the hunt for an experienced Adults Social Worker to lend their expertise to an ongoing Section 117 project. You will work alongside health colleagues to carry out joint aftercare reviews, ensuring tailored and person-centred support for individuals discharged from mental health detention. Your role is pivotal in making sure these individuals continue to receive the appropriate care needed.

What you will do

• Undertake Section 117 aftercare reviews in collaboration with NHS and health teams.
• Regularly assess eligibility and effectiveness of current S117 aftercare arrangements.
• Cooperate with service users, carers, providers, and partner agencies to craft detailed support plans.
• Review care packages and propose improvements for the most supportive care environments.
• Keep case records and assessments up to date and accurate.
• Apply relevant mental health legislation in your daily practices, ensuring compliance with statutory guidance.
• Engage in multi-disciplinary meetings, fostering collaborative partnerships across health and social care sectors.
• Tackle a caseload of complex mental health cases, fulfilling all statutory obligations.

Essential Requirements

To succeed in this role, you must be a Qualified Social Worker registered with Social Work England, bringing significant experience within Adult Social Care. Having prior experience with Section 117 aftercare reviews, a robust understanding of the Mental Health Act, and the ability to manage intricate case loads will be crucial. Strong communication, assessment, and report-writing skills are essential for effective collaboration with NHS and multidisciplinary health teams.
