What you will do
- Provide specialist clinical pharmacy input, including medicines reconciliation, clinical screening, and therapeutic drug monitoring.
- Review and optimise medication for patients with acute and long-term conditions, ensuring safe and effective prescribing.
- Collaborate with consultants, nursing staff, and other members of the multidisciplinary team to support high-quality patient care.
- Promote medicines optimisation and ensure that prescribing practices comply with relevant clinical guidelines, policies, and governance standards.
- Offer professional support and guidance to pharmacy colleagues, including pharmacists and pharmacy technicians.
Requirements
- Current GPhC registration as a Pharmacist.
- Strong post-registration clinical pharmacy experience.
- Previous hospital pharmacy experience is highly desirable.
- Excellent knowledge of medicines optimisation, clinical screening, and medicines reconciliation.
- Ability to work confidently and effectively within a multidisciplinary clinical environment.
