What you will do

• Undertake comprehensive mental health, cognitive, risk, and capacity assessments for older adults referred to the service.
• Manage a complex caseload and act as Care Coordinator, developing and reviewing person-centred care and treatment plans.
• Contribute to memory assessment pathways, dementia diagnosis processes, and post-diagnostic support for individuals and families.
• Deliver evidence-based psychosocial and therapeutic interventions to support recovery, wellbeing, and independence.
• Work collaboratively within multidisciplinary team meetings and contribute to clinical decision-making and case formulation.

Person Specification

Qualifications

• Registered Mental Health Nurse, Occupational Therapist, or Social Worker.
• Professional qualification recognised within the UK healthcare sector.
• Evidence of post-registration study at Degree, Diploma, or equivalent level.
• Practice Assessor, Practice Supervisor, or Mentorship qualification is desirable.

Registration & Compliance

• Current registration with NMC, HCPC, or Social Work England, as appropriate.

Experience

• Significant post-registration experience within community mental health, older adult mental health, or memory services.
• Experience supporting individuals with dementia, cognitive impairment, and complex mental health needs.
• Demonstrable experience undertaking autonomous assessments, care coordination, and crisis/risk management.
• Previous involvement in supervision, service development, or quality improvement initiatives is advantageous.
