What you will do:

• Monitor long-term CHC provision, ensuring care plans are effective and meet the needs of individuals.
• Complete joint reviews, working collaboratively with health colleagues to provide holistic care solutions.
• Engage in safeguarding activities, ensuring the protection and safety of vulnerable adults.
• Undertake assessments in line with the Care Act 2014 to determine eligibility and care needs.
• Participate in Decision Support Tool Meetings to support decision-making processes.
• Handle Court of Protection work, advocating for individuals requiring legal support.
