What you will do

• Undertake Care Act assessments, reviews, support planning and safeguarding enquiries for adults with eligible care and support needs.
• Manage a complex caseload, ensuring assessments and interventions are proportionate, strengths-based, and focused on achieving positive outcomes.
• Review care packages and commissioned services to ensure effectiveness, appropriateness and best value.
• Promote independence through preventative interventions, community resources, assistive technology and reablement opportunities.
• Work collaboratively with multidisciplinary teams including NHS professionals, mental health services, housing providers and voluntary sector partners.

Person Specification

Qualifications

• Possess a Degree in Social Work, CQSW, DipSW or equivalent recognised social work qualification.
• Successfully complete the ASYE (Assessed and Supported Year in Employment).
• Present evidence of continuing professional development.

Registration and Compliance

• Hold current registration with Social Work England.

Experience

• Have previous experience within UK Adult Social Care.
• Demonstrate experience undertaking Care Act assessments and reviews.
• Possess experience working with vulnerable adults, carers and complex safeguarding situations.
• Exhibit a proven ability to work within multidisciplinary environments and deliver outcome-focused interventions.
