What you will do

• Screen and assess new referrals and contacts received by Adult Social Care, playing a crucial role in identifying the initial needs of individuals.
• Gather and analyse information from individuals, families, professionals, and partner agencies to make well-informed decisions.
• Conduct strengths-based conversations and carry out proportionate assessments to determine needs, risks, and desired outcomes.
• Apply key pieces of legislation such as the Care Act 2014, Mental Capacity Act 2005, and safeguarding adults procedures in everyday practice.
• Identify and manage safeguarding concerns using local threshold guidance, including making recommendations for Section 42 inquiries where needed.
• Provide top-notch information, advice, and signposting to individuals and carers, supporting independence and preventing further escalation of needs.
• Carefully assess levels of risk to safeguard adults with care and support needs.
• Effectively liaise with health partners, providers, voluntary organisations, and other agencies to ensure timely and coordinated responses.
• Maintain accurate, timely, and defensible records, adhering to organisational requirements.
• Contribute enthusiastically to duty functions and help manage workflow effectively within the Access Team.
