Collaborate closely with health colleagues, monitoring long-term CHC provision and ensuring quality service delivery.
Complete joint reviews and assessments in line with the Care Act 2014, ensuring support is tailored to meet individual needs.
Engage in Decision Support Tool Meetings to facilitate appropriate care planning.
Participate in Safeguarding and Court of Protection work to uphold the rights and interests of adults within the community.
Conduct thorough assessments and evaluations to support decision-making and care provision.
