What you will do:

- Conduct thorough assessments in accordance with the Care Act 2014, along with carers assessments.
- Develop and execute bespoke, person-centred support plans.
- Manage a diverse caseload, including handling safeguarding cases with care and precision.
- Engage collaboratively within multi-agency settings to foster comprehensive care solutions.
- Empower service users by facilitating access to community, voluntary, and broader services.
- Participate actively in a duty rota, efficiently managing referrals and offering immediate advice.
- Complete mental capacity assessments in compliance with current legislation.
