What you will do:
- Undertake clinical pharmacy duties focusing on the care of elderly rehab patients.
- Conduct thorough medicines reconciliation and advise on medication reviews.
- Provide efficient TTAs (To Take Away) preparation and checking.
- Offer minimal dispensary cover, ensuring accurate dispensing and patient safety.
- Collaborate with multidisciplinary teams to optimise pharmaceutical services.
- Participate in ward rounds and contribute to clinical meetings.
Requirements:
- Current registration with the General Pharmaceutical Council (GPHC).
- Proven experience working as a Pharmacist within a UK NHS pharmacy setting.
- Qualified with NVQ Level or its equivalent in Pharmacy.
- Proficient in medicine reconciliation, clinical pharmacy practices, and preparing TTAs.
