What you will do:
- Conduct assessments to identify equipment and environmental adaptation needs, aiming to deliver comprehensive, person-centred rehabilitation programmes within patients' homes.
- Collaborate closely with physiotherapists to develop and implement effective care plans.
- Supervise and support patient support workers, ensuring high standards of care and continuity.
- Use your expert communication skills to liaise with patients, families, and other healthcare professionals, ensuring everyone is informed and involved in rehabilitation processes.
- Manage a varied caseload, bringing your organisational skills to deliver optimal care and treatment outcomes.
Requirements of the Occupational Therapist:
- Degree or equivalent in Occupational Therapy
- Recent UK experience working as an Occupational Therapist, including NHS experience
- Registered with the HCPC
- Proven ability to work holistically, focusing on person-centred rehabilitation
- Strong collaboration skills and experience working alongside physiotherapists and managing patient support workers
