Join us for an exciting ongoing opportunity as a Band 7 Mental Health Practitioner within the dynamic setting of Tandridge GP Surgeries in Caterham, Surrey. Be part of a team that's making a genuine difference, as you support the development and mobilisation of integrated primary care mental health services. The role offers a structured schedule, working Monday to Friday, from 9 AM to 5 PM. You will be providing early assessment, evidence-based interventions, and fostering strong community support links for individuals with serious mental illness, while situated within both North Tandridge and South Tandridge Primary Care Networks (PCNs).

What you will do:
- Conduct comprehensive mental health assessments and provide evidence-based interventions to patients with serious mental illness.
- Collaborate with GPs and other healthcare professionals to deliver integrated care plans.
- Develop and implement strategies to improve mental health services within the primary care setting.
- Engage with community resources to enhance support networks for patients.
- Supervise and mentor junior staff, fostering a culture of continuous learning and improvement.
