What you will do:

  • Become an integral member of the Brent CMHT, leading primary care mental health liaison and link working.
  • Conduct initial and comprehensive health and social care assessments, either independently or collaboratively, and communicate findings back to the Multi-Disciplinary Team.
  • Handle care coordination and co-worker responsibilities, managing the care of a specific group of service users with a commitment to recovery and social inclusion.
  • Provide supervision and support to a designated group of staff and students within the team.
  • Participate in MDT activities, including duty and clinic rotas, and strengthen communication between Mental Health Services and General Practice Teams.
  • Uphold CNWL policies and procedures, actively aligning with the organisation’s values and ethos.
  • Maintain regular contact with GP Practices as part of the link worker role, enhancing mutual understanding of CMHT functions.
  • Collaborate with other multi-disciplinary team members to deliver comprehensive MH care pathways.
  • Educate health professionals about secondary mental health service criteria, aiding in referral triage and allocation.
  • Plan and implement care strategies tailored to patient needs based on best practice guidance.
  • Continuously review and adjust care and risk management plans.
  • Provide medication management advice, and carry out physical and mental health reviews when necessary.
  • Engage with primary care to facilitate timely and appropriate referrals to secondary services.
  • Work within various settings including base, clinics, and home environments as needed.
  • Liaise with community and voluntary agencies for patient support expansion.
  • Ensure smooth transition and discharge back to primary care as appropriate.
  • Actively participate in and coordinate training events and meetings within the organisation.
