What you will do:

  • Undertake comprehensive mental health assessments and develop person-centred care plans in line with Care Programme Approach (CPA) and recovery models
  • Manage a defined caseload as a Care Coordinator, overseeing care delivery, reviews, and discharge planning
  • Deliver evidence-based interventions, including risk management, crisis response, and therapeutic support in community settings
  • Work collaboratively within a multidisciplinary team including psychiatrists, social workers, and occupational therapists

Qualifications:

  • Registered Mental Health Nurse (RMN) qualification
  • Current NMC registration

Registration & Compliance:

  • Adherence to the NMC Code of Professional Conduct
  • Knowledge of the Mental Health Act, Mental Capacity Act, and safeguarding frameworks
  • Understanding of NHS governance, CQC standards, and clinical audit requirements
