What you will do
- Deliver high-quality community-based physiotherapy interventions within patients' homes.
- Work collaboratively within an integrated health and social care service alongside Occupational Therapists and support staff.
- Apply your rehabilitation and orthopaedic experience to support patient recovery and maximise independence.
- Assess, recommend, and provide minor equipment to support daily living and mobility needs.
- Conduct holistic assessments to identify and address wider health and wellbeing needs.
- Complete falls risk assessments and implement appropriate prevention and management plans.
- Develop and review personalised treatment plans to achieve positive patient outcomes.
- Maintain accurate clinical records and documentation in line with professional standards.
- Promote independence, functional mobility, and quality of life through evidence-based interventions.
Requirements
- HCPC Registration: Current registration as a Physiotherapist with the Health and Care Professions Council.
- Driving Licence: Full UK driving licence essential for community visits.
- Community Experience: Previous experience within community physiotherapy services is desirable.
- Rehabilitation Expertise: Experience in rehabilitation and orthopaedic physiotherapy.
- Assessment Skills: Strong clinical assessment and treatment planning abilities.
- Communication Skills: Excellent interpersonal skills with the ability to work effectively with patients, families, and multidisciplinary teams.
