What you will do:

- Provide statutory support to adults with care and support needs and their carers or families, including assessment, support planning, and safeguarding.
- Comply with legal frameworks for social care, putting individuals and communities at the heart of care and wellbeing.
- Undertake Mental Capacity Assessments (MCA) and make best interest decisions following MCA 2005 principles.
- Collaborate with partners in health care, private providers, and the voluntary sector to coordinate support for individuals and carers.
- Maintain accurate and timely records of all actions and information on the social care database.
- Conduct Section 42 (Care Act) enquiries and work in multi-agency partnerships according to Swindon safeguarding policies.
