About the role:

As an Adults Social Worker within the hospital setting, you will collaborate with healthcare and social care professionals to provide high-quality, person-centred support. Under the Home First philosophy, you will promote independence and recovery, ensuring individuals receive essential support both during their stay and after returning home.

What you will do:

• Conduct person-centred assessments with patients and family members.

• Develop and implement care plans that foster independence and support wellbeing.

• Coordinate with multidisciplinary teams to ensure seamless discharge planning.

• Provide follow-up support and reviews post-discharge at homes or care facilities.

About you:

• A deep understanding of the Care Act 2014 and its practical application.

• Experience in conducting Mental Capacity Assessments and acting in individuals’ best interests.

• Familiarity with Continuing Healthcare reviews.

• Excellent communication and organisational skills to thrive in a busy environment.
