What You Will Do:
- Complete Assessments: Undertake person-centred assessments with patients and families to identify needs and support requirements.
- Develop Care Plans: Create and implement tailored care plans that promote independence, wellbeing, and positive outcomes.
- Facilitate Safe Discharges: Work alongside multidisciplinary professionals to coordinate timely and effective discharge arrangements.
- Provide Ongoing Support: Assist individuals following discharge, reviewing and adapting support plans as required.
- Promote Collaborative Care: Act as a key point of contact for healthcare professionals, service users, and families to ensure coordinated support.
Requirements:
- Qualified Social Worker with Social Work England registration.
- Experience working within Adult Social Care settings.
- Comprehensive understanding of the Care Act 2014.
- Knowledge and experience of Mental Capacity Assessments and Best Interest decision-making.
- Understanding of Continuing Healthcare reviews and processes.
- Excellent communication, organisational, and case management skills.
- Ability to work effectively within a fast-paced multidisciplinary environment.
