What you will do:

- Provide dedicated support to clients with substance misuse issues, focusing on both harm reduction and recovery models.
- Manage a caseload of clients, offering keyworking services to empower them through their recovery journey.
- Facilitate group sessions and one-on-one meetings to promote positive behavioural changes and increase awareness.
- Collaborate with multidisciplinary teams to provide comprehensive care plans tailored to individual client needs.
- Maintain accurate records and reports, ensuring timely updates for ongoing case management.

Requirements:

- A DBS check on the update service.
- Proven experience working with substance misuse cases involving drugs and alcohol.
- Strong skills in caseload management and keyworking, with a proactive approach to handling complex cases.
- A solid understanding of harm reduction strategies and recovery models.
