What you will do

• Manage a caseload of young people transitioning into adulthood, providing essential guidance and support
• Undertake assessments, reviews, and care planning in accordance with legislative and organisational requirements
• Collaborate with individuals, families, carers, and multi-agency professionals to develop holistic support plans
• Champion independence, choice, and control through strengths-based practice
• Safeguard vulnerable adults, ensuring risks are appropriately assessed and managed
• Maintain accurate and timely case recordings and documentation
• Positively contribute to team development and service improvement initiatives

Essential Requirements

To shine in this role, you must:

• Hold a recognised Social Work qualification and be registered with Social Work England
• Have a minimum of two years' post-qualification experience in social work practice
• Demonstrate experience of using LAS (Liquidlogic Adult System)
• Possess a sound understanding of relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005
• Boast strong assessment, communication, and case management skills
• Hold a valid driving licence and have access to a vehicle for work purposes
• Reside within one hour's travelling distance of Rotherham
