As the role involves travelling across Pembrokeshire, a valid driving licence and access to a vehicle are essential.

What You Will Do
- Provide high-quality physiotherapy care to patients within community settings.
- Manage a varied community caseload, supporting initiatives aimed at reducing community hospital waiting lists.
- Conduct comprehensive patient assessments and develop personalised treatment and rehabilitation plans.
- Collaborate with a multidisciplinary team, including healthcare professionals across community and hospital services.
- Support patients in improving mobility, independence, and functional outcomes through evidence-based physiotherapy interventions.
- Travel across Pembrokeshire to deliver physiotherapy services within community settings.
- Maintain accurate clinical documentation and patient records in line with professional standards and service requirements.
Requirements
- Degree or equivalent qualification in Physiotherapy.
- Current HCPC registration as a Physiotherapist.
- Experience working within community or rehabilitation physiotherapy services is desirable.
- Strong assessment, treatment planning, and clinical reasoning skills.
- Ability to work independently while contributing effectively within a multidisciplinary team.
- Full UK driving licence and access to a vehicle due to travel requirements across Pembrokeshire.
