Role Overview

An exciting opportunity has arisen for a Social Worker to join the Hospital Discharge Team in Brent. This ongoing locum position offers the chance to play a key role in supporting adults transitioning safely from hospital to community-based care.

Working within a fast-paced hospital setting, you will assess individuals’ needs, coordinate care plans, and collaborate with healthcare professionals to ensure safe and effective discharges. This role provides a rewarding opportunity to support individuals in regaining independence while ensuring appropriate support systems are in place.

Following an initial induction period working on-site at the hospital, the role offers the flexibility of hybrid working, with the opportunity to work remotely for part of the week.

What You Will Do

  • Conduct assessments for adults recently discharged from hospital to identify care and support needs.

  • Develop and implement person-centred care plans to support safe discharge and recovery within the community.

  • Work closely with healthcare professionals, including hospital staff and community services, to coordinate care packages.

  • Attend the hospital regularly during the initial period to develop an understanding of processes and build relationships with multidisciplinary teams.

  • Provide guidance and support to individuals and their families during the transition from hospital to home or community care.

  • Advocate for service users, ensuring they have access to appropriate services and resources.

  • Maintain accurate case records and documentation in accordance with statutory and local authority requirements.

Requirements

  • Qualified Social Worker with a recognised Social Work qualification (BA/MA Social Work, DipSW, or equivalent).

  • Current registration with Social Work England.

  • Experience working within Adult Social Care, ideally within hospital discharge or community-based teams.

  • Strong understanding of relevant legislation including the Care Act 2014 and safeguarding procedures.

  • Ability to work effectively within multidisciplinary teams in a fast-paced environment.

  • Excellent communication, assessment, and organisational skills.
