What you will do:

Manage your own caseload involving Adult Social Care tasks such as home assessments, recommendations, and equipment provisioning.

Conduct manual handling assessments, reviews, and provide specialist seating assessments.

Support reablement assessments and reviews to recommend care package adjustments.

Prepare specifications and diagrams for major adaptations like wet rooms, stairlifts, and ramping.

Provide duty support with equipment management alongside social workers and duty managers.

Facilitate welfare contacts through phone and home visits.

Collaborate with various health and social care teams including NRS, IIT, ACRT, and CMHT.

Attend monthly meetings that focus on business and development.

Engage in project work and conduct housing assessments and recommendations.

Conduct telecare assessments and collaborate with responders like the Camden care line.

Utilize databases and software like Mosaic, Word, and Excel effectively.

Be open to out of borough visits as required.
A candidate who is able to adapt to change and willing to take on additional tasks for team support is essential. An enhanced DBS check is required, and you should be a registered driver with a valid HCPC license.
