What you will do:

• Conduct comprehensive assessments of service users experiencing a wide range of mental health needs.
• Plan, implement, and regularly review care pathways and care plans, particularly for individuals with complex mental health presentations.
• Monitor the use and administration of psychotropic medication, including Clozaril and Lithium, ensuring safe practice and adherence.
• Undertake comprehensive risk assessments and implement appropriate risk management plans to promote the safety and wellbeing of service users and others.
• Explore the impact of physical health, social circumstances, and wider support needs on mental health and wellbeing.
• Participate in the rotational Duty System and provide support as a back-up Duty Officer when required.
• Work collaboratively within a multidisciplinary team to deliver effective, recovery-focused interventions.
• Build positive therapeutic relationships with service users, families, and carers.
• Contribute to coordinated care planning and ensure service users receive appropriate support throughout their care journey.
• Promote innovative and evidence-based approaches to community mental health practice.
• Maintain accurate clinical records and work in accordance with relevant professional standards, policies, and procedures.

Essential requirements:

• Core professional registration as a Mental Health Practitioner.
• Current registration with the relevant professional body.
• Post-qualification experience within mental health services.
• Experience working across diverse clinical settings.
• Strong assessment, care planning, and risk-management skills.
• Experience working with complex mental health presentations.
• Knowledge and experience of psychotropic medications, including Clozaril and Lithium.
• Ability to work effectively within a multidisciplinary team.
• Strong communication and interpersonal skills.
• Ability to work independently while contributing positively to a collaborative team environment.
