What you will do:
- Champion positive outcomes for young people by supporting and empowering them, their families, and carers.
- Help young people develop independent living skills at the right time and in a way that reflects their individual strengths and aspirations.
- Complete support conversations and Care Act assessments consistently and effectively.
- Develop personalised support plans that reflect identified needs, strengths, and desired outcomes.
- Lead the commissioning process for appropriate services where assessed needs require additional support.
- Explore creative alternatives to commissioned services, including assistive technology, community assets, and other forms of non-traditional support.
- Promote independence by helping young people and their families access appropriate community-based resources.
- Contribute to the Early Help model within the Transitions Hub.
- Work collaboratively with young people, families, carers, and partner agencies to ensure coordinated support.
- Maintain accurate records and ensure assessments and support plans are completed to a high professional standard.
- Promote strengths-based, person-centred practice throughout the transition to adulthood.
Essential requirements:
- Relevant Social Work qualification and current Social Work England registration.
- Experience working as a Social Worker within Adult Social Care.
- Understanding of transitions for young people moving into adulthood.
- Knowledge of the Care Act and experience undertaking Care Act assessments.
- Strong assessment, care planning, and case-management skills.
- Ability to work effectively with young people, families, carers, and multidisciplinary partners.
- Strong communication, relationship-building, and professional decision-making skills.
- Commitment to strengths-based and person-centred social work practice.
